What makes an effective Performance Management System?

Human Resources staff often tell us that leaders and staff experience little benefit from their annual performance review discussions.
Leaders question the relevance of the many "forms" they are required to complete and the balance given to performance assessment vs personal development in these reviews. Leaders are slow to schedule discussions with staff and to complete the necessary "paperwork"  required to finalise these reviews.

It seems that no-one is asking the important questions:

Staff report that the most important element of the system is the time and coaching they receive from their direct manager.
